Supporting Community Members Independence and Quality of Life

Our Home and Community Care program is facilitated by our Home and Community Care Nurse (HCCN) and our Personal Support Workers (PSW).

What does HCC Offer?  The Home and Community Care program provides quality home support and community care services to the community in a respectful, compassionate, and empathetic manner. These supports include helping maintain the unique physical, emotional, social and spiritual needs of the individual and his/her family. The program provides Personal Support Workers in the home to aid in the quality of life for the client and also for the social companionship that is needed. We also have programs to support the physical needs with a loan cupboard to lend equipment needed to clientele and community members.

The goals of this program are to:

  • Support community members to maintain their independence, quality f life, and safety within their home
  • Provide nursing services offering case management and health teaching to clients and caregivers for therapeutic interventions, treatments, and procedures
  • Offer PSW services to assist clients with personal care, household chores and management, appointments and transportation, meal prep and grocery shopping, and respite care
  • Mobility and safety devices available for loan

Cathy Banks, RN is the Nurse Manager for the program and oversees the program delivery and ensures that all the needs are met within the community based on a client-centered approach. The nurse manager is responsible for ensuring that clientele have access to services, program elements and standards are maintained, ensuring that staff are continuously updating their skills and provide confident services with a strict confidentiality agreement with program and clients.
Jodi Bradfeild, is the Registered Practical Nurse that offers nursing care to the clients of the program. This roll also provides health teachings and management skills to Home and Community Care staff and clients. Performs ongoing health and needs assessments of clients; ensuring services are provided to assist with care and to promote independent living. Assist with treatments and rehabilitation to prevent deterioration and promote wellness.
Jaqueline Johnson, Community Support Service Coordinator is responsible for program delivery with focus on the social needs of elderly within the community. This program provides clients with programs focused on falls prevention classes, congregate dinning, social gatherings, outings, exercise programs and shopping trips. Some programs implemented are swimming with YMCA, Bowling in Owen Sound, Shopping trips, farmers market trips, festival of lights, etc.
Personal Support Workers: Home and Community Care currently has 13 PSW staff to meet the needs of the elderly or people with disabilities in their homes. The PSWs offer one and one support to their clients with focus on helping them manage their quality of life through personal care and household management, preparing meals, and help in maintaining their social needs. Our staff are all highly trained and very compassionate and empathetic to their client needs.
Cherylyn Kitchikake, Office Administration/PSW offers support to all staff within the office and also with the clients and the home setting. This role also provides transport services to the clients with the elder’s wheelchair accessible bus and/or van to community programs and/or CSS programs. This role is unique as it provides support to all aspects of the program with client, psw support, and office management staff.
