Supporting Community Health, One Person at a Time

This program is facilitated by the Community Health Representative (CHR) and the Community Health Nurse (CHN).

Our Community Health Staff provide health promotion, health maintenance, injury and illness prevention within the community to all ages and stages.  The programs and supports are offered through community engagement, awareness and education, within a group format or on an individual basis.
